Nurse Practitioner or Physician Assistant

Life Transitions is seeking a Nurse Practitioner or Physician Assistant to fulfill the organization's role of evaluating and coordinating patient/carefiver needs to enable the delivery of palliative care.

Nurse Practitioner must be a graduate of an accredited school of nursing with a Master's degree in nursing and appropriate advanced training required for licensure as a registered practitioner. Two years experience with hospice and home health preferred.

Physician Assistant must be a graduate of an accredited Physician Assistant program and appropriate advanced training required for licensure as a physician assistant. Must be board certified by the National Commission on Certification of Physician Assistants (NCCPA) and maintain DEA license to prescribe. Two years experience with hospice and home health preferred.

Education:

Graduate of an accredited school of nursing with a Master’s degree in nursing and appropriate advanced training required for licensure as a registered nurse practitioner.  Current, unrestricted North Carolina Nurse Practitioner license.  Must maintain CPR certification.

Experience:

A minimum of two to ten years of nursing experience preferred.  Hospice or home health experience preferred.

Physical Requirements:

  • Must be able to perform basic life operation skills of sitting, talking and hearing.
  • Must be able to perform sedentary work; and to lift/move objects up to 50 pounds and be able to support, move, lift, transfer or reposition patients.
  • Must possess the visual acuity to operate a computer terminal and to read and write extensively.

Work Environment (High Risk)

Person in this position could be at risk for exposure to infections, communicable diseases, odors, blood, excreta, and/or hazardous materials.  Employee will be working with terminally ill patients and/or their caregivers, which can cause periods of stress, variable workloads, flexible hours and deadlines.  Must be able to travel within the community up to 75% of the time.
