Levels of Care

Healthcare Providers side by side.jpg

One of the reasons hospice care is so effective is that it focuses on the patient and addresses all aspects of a person - body, mind, and spirit. Hospice support also extends to the family and to caregivers.

Expertise from a specialized health care team offering different levels of care work together to help patients live their best life.

Offering 4 Levels of Care

Routine Home Care

Offered wherever you call home including hospitals, nursing homes, assisted livings or your personal home.

General Inpatient Care

Short-term, 24-hour care is available in our hospice houses for a short time to manage acute patient symptoms that cannot be managed in other settings, such as medication adjustments, observations, or stabilizing treatments. This service must be provided according to Medicare guidelines and is covered by Medicare, Medicaid, and many insurance companies. Patient admission is dependent on bed availability.

Respite Care

Inpatient short-term care is available at our hospice houses so that family members or caregivers can get a break from demands of caregiving. Respite care is available for five days per respite episode. This service must be provided according to Medicare guidelines and is covered by Medicare, Medicaid, and many insurance companies. Patient admission is dependent on bed availability.

Continuous Care

A higher level of care provided in the home by a dedicated continuous care team in periods of crisis.

It is important that patients and families understand and accept criteria for each level of care prior to an admission.
